Financial Aid Forms

You only need the Free Application for Federal Students Aid (FAFSA) to apply for financial. After you submit the FAFSA, however, you may be asked to complete one of the forms below. Please do not submit these forms unless you are advised to do so by the Office of Financial Aid.

	Identity & Statement of Educational Purpose Form (Aug 2017 - July 2018) This form is required only for selected students to confirm the student's identity and intention to use financial aid only for educational purposes Fewer than 1% of students are required to complete this form, which must be completed and signed in the presence of a notary public In lieu of a notary, you may complete and sign this form in-person in the presence of a Financial Aid Counselor at Harding University This form is an adobe acrobat file
	Verification Worksheet (Aug 2016 - July 2017) About 30% of FAFSA applicants are chosen for verification Required for all applicants selected for verification by the FAFSA processor

	IRS Form 4506-T Use form to request Verification of Nontax Filing or a free Tax Transcript Form may be used to request documents mailed directly to the financial aid office
	Untaxed Income Worksheet (Aug 2016 - July 2017) This form is required only for selected households with untaxed income received or applicants with unusually low incomes This form is an adobe acrobat file
	Supplemental Verification Worksheet (Aug 2016 - July 2017) This form is required only for selected households with child support paid or food stamps (SNAP) received This form is an adobe acrobat file

	Parent PLUS Loan Credit Balance Authorization Parent borrowers complete this optional form after applying for Parent PLUS Loan Use this form to indicate if extra funds from the PLUS loan should be refunded to the student, to the parent, or returned to the lender to reduce the amount borrowed This form is NOT used to apply for a PLUS loan.
